Single Page Order Form

Place your order using our long-standing single page order form, which we are offering because many of our customers prefer to see everything at a glance when ordering.

Please note the single page order form is a stand-alone order form. In other words, if you add food to your shopping cart elsewhere on the website, this will not carry over to an order on the single page order form. If you want to use the single page order form, you should make a complete order on that page.

Order by Phone

Order by Fax

First, make your selections on our single page order form. During checkout, you will be given four payment options. Select "Order by Credit Card using FAX" to pay by Visa or MasterCard via FAX. After you click "Submit Secure Order" a page will appear that you can print out and FAX to us at 888-363-8310 or 831-457-0158. Please remember to complete the credit card information and sign the form before faxing.

Order by Mail

First, make your selections on our single page order form. During checkout, you will be given four payment options. Select either "Order by Check/Money Order through Postal Mail" or "Order by Credit Card through Postal Mail". After you click "Submit Secure Order" a page will appear that you can print out and mail to us along with your check/money order or credit card information.

Shipping Options

Shipping Via Other Methods

If you want to ship your order by any other method than FedEx Ground Residential or UPS Ground (such as overnight, 2nd day air or 3-day select) and any USPS option, you are responsible for all freight charges. Our shopping cart interfaces with the current rate charts of UPS, FedEx and USPS; this gives you the opportunity to see your rates using various methods. The shopping cart then provides the appropriate freight charges based on the estimated weight of your order to your specified domestic zone or country after you press the checkout button.

UPS and Fed Ex rates are comparable within the continental United States.

Outside Continental U.S.

For people outside the 48 Continental United States (including Alaska, Hawaii, Puerto Rico and the Virgin Islands), the most cost-effective option is usually USPS Priority Mail International which also offers tracking for your package. USPS also offers Global Express Guaranteed® and Express Mail International® as alternative shipping methods with a tracking option. You are responsible for all international freight charges.

Insulated vs. Non-Insulated Shipping

NOTICE: If you are ordering any of our Raw Cultured Vegetables, please read the following information carefully.

We have done research that indicates that during shipping, shelf-life is maximized and expansion of Rejuvenative Foods is minimized when salt-free Raw Cultured Vegetables (when unrefrigerated) are maintained at a temperature lower than 74 degrees for 9 days or less. And similarly, shelf-life is maximized and expansion is minimized for salted Raw Cultured Vegetables (when unrefrigerated) are maintained at a temperature lower than 77 degrees for 9 days or less. This temperature information can be found on our Raw Cultured Vegetable labels.

We ship your Raw Cultured Vegetables either insulated or non-insultated depending on whether you want to spend $45 on insulation, and whether you are happy receiving expanded and/or leaky cultured vegetables (the insulated shipments may arrive with some expansion and leakage but less than non-insulated). This idea is explained below.

If you are placing an insulated order: You can expect the Rejuvenative Foods Raw Cultured Vegetables to arrive with a diminished potential for expansion compared to a non-insulated order. The enhancement achieved by natural expansion is still likely to occur even though you are ordering an insulated shipment. Furthermore, even though your order will be shipped out insulated with coldpacks, by the time it gets to you there is a fair chance it will no longer be cold. There is also a fair chance that condensation will create extra moisture on the labels; in fact, you may end up with completely wet labels, as the jars are protecting the delicious raw cultured vegetables within them.

If you are placing a non-insulated order: The Raw Cultured Vegetables are likely to be enhanced through natural expansion, which is fine, although there is a potential for achieving a longer shelf life by ordering them insulated instead (for $45). You will see the $45 insulated shipping option on the order form page.

Please complete this order in conjunction with the understanding that if your shipment arrives leaky and/or expanded, it will still meet your expectations. And please remember that our money back guarantee is limited to 1 jar per flavor (non bulk 15 or 7.5 oz) up to 6 flavors total.
